to be reviewed and filled out as appropriate(adult and/or youth specific clients) prior to visiting Wentworth & Associates,
P.C.
Please fill out the required forms and questionnaires at your convenience and bring
them all with you to your first appointment. This will
help your therapist to understand you and save a great
deal of time.
If you do not feel comfortable answering any questions,
please leave it blank. You are never required to answer
any questions. You may simply wait until your appointment
and discuss it with your therapist.
If you cannot download the forms, we can send them to
you in the mail or arrange to have them available an hour
before your appointment to fill out in the waiting room.
